Recognition Criteria

Students are nominated by the school that they attend. Five students from each school in Madison Heights will be recognized at the ceremony.

Nominations MUST be received by MARCH 24, 2023

There are three different ways to submit a student nomination form:

1. Complete the online nomination form below.

2. Print, complete, and send the nomination form to:
